Job description:
An opportunity has arisen for an experienced nurse or allied health professional to join our pro-active care team. We strongly encourage applicants from a therapy or dementia background. This is an exciting and challenging opportunity to assist in the development of this model of care, requiring forward thinking, motivation, enthusiasm and flexibility. The role is varied, with a key focus on preventing unnecessary hospital admissions. Utilising caseload management skills, clinical competencies, and advanced nurse practitioner knowledge, the community matron will work in partnership with the patient, carer and existing community services in Health and Social Care, to negotiate and agree future management plans which will help enable the patient to independently manage their own condition and or prevent a crisis.
