Job Number: HC04727 Division: Health Care Location: Feltham Contract Type: Permanent Schedule Type: Full Time Closing Date: 21-Mar-2016 Job Summary Opportunity has arisen for a passionate Mental Health Nurse to join our Inpatients team at HMYOI Feltham. You'll provide a holistic package of care to patients within the prison system, managing the assessment of health needs; planning, delivery and evaluation of care, for the prison population, including clinical risk assessment. HMYOI Feltham is a Young Offenders Institute for young people aged 15-18 and young adults aged 18-21. Responsible To Clinical Lead Key Responsibilities •Work with the team, ensuring the delivery of high quality, individualised, evidence-based, patient centred nursing care •Direct and supervise the work of nursing assistants in meeting identified health needs •Assess physical and mental health of prisoners on admission to the prison so that any appropriate referral can be made. This involves consideration of a range of options for accommodation for the prisoners’ first night in prison •Participate in the delivery of holistic care in the Department whilst working closely with a multi-disciplinary/professional team. •Administer medication as prescribed by the doctor within the guidelines for medicines administration. •Assess and treat patients with minor ailments and injuries or refer to the appropriate unit. •Provide nurse led care along the guidelines of the National Service Framework by running regular clinics, for example: Phlebotomy, Vaccinations, Smoking Cessation, Secondary Health Care Screening. •Respond to medical emergencies within the prison. Carrying emergency equipment to prison wings to provide first on the scene response for resuscitation, CPR, defibrillation as necessary. •Identify those that are at risk of self harm and put in place plans to reduce said risks. •Open, maintain, close and take part in reviews of individuals being managed under the Prison Suicide and Self Harm risk management system known as Assessment, Care in Custody and Teamwork, (ACCT). •Ensure that Safer Cell procedures are followed and monitor patients on constant supervision where they are considered particularly at risk from suicide or self harm, recording observations. •Maintain accurate records of care provided on SystmOne, based upon the care plan and incorporating all relevant communications and liaison. •Positively adopt all relevant Care UK and Home Office policies, protocols and procedures and action these as necessary. •Work within the Clinical Governance Framework. •Report all risks to the senior nursing staff and prison security staff. •Maintain and update relevant knowledge and skills, maintain a professional portfolio and participate in further training as appropriate. •Take responsibility for carrying a radio and responding as required. •Take responsibility for assessing the mental and physical health of prisoners contained in the Care and Separation Unit on a daily basis. •Attend internal and external meetings concerning the care of the prisoners. •Be conversant with and use the electronic clinical records system, SystmOne and pNOMIS. •Work closely with the Healthcare Management to ensure that the Prison Health Agenda is supported at all levels in the Practice. •The clash of cultures means that sensitive, complex and contentious issues need to be communicated with sensitivity and empathy. The post holder will need to present new ideas in a persuasive manner. •Excellent communication skills are vital to ‘hand over’ to the next nurse in charge.
