Support Worker - Community Connections (Full & Part Time) Cardiff £14,657.76 - £14,861.34 per annum plus benefits We have exciting opportunities to join our Cardiff Community Connections Team as a Support Worker. You will join a team working across Cardiff providing support to individuals with disabilities in their own home. Our Community Support Service works in partnership with people we support, their families and specialist teams to provide consistent, citizen directed support services. We support people to make own choices, develop their skills to enable them to live as independently as possible within their own homes, develop the relationships that are important to them, and be an active part of their local communities. Minimum 6 months experience in social care and using your own car with a valid driving licence and business insurance is essential. This role provides an ideal opportunity to develop knowledge and experience within the field of learning disabilities. It is essential that you are able to work as part of a team. These posts require a firm commitment to enabling people to maintain present skills, develop new ones and build their self belief and confidence. You must have a positive attitude and be able to bring passion and energy to this role in order to achieve this. The ability to follow detailed plans is essential. We will ensure that you will receive the necessary training and induction. Work duties will include supporting people with: Activities in the community – attending a range of structured or unstructured activities medical appointments, social events, shopping, etc. Personal care –this may include washing, bathing and assistance with toileting, etc. Practical support –Completion of domestic duties such as laundry, cleaning, cooking meals, etc. Specialized Care –Complex needs such as Challenging behaviour, long term health condition, etc. Office work – updating plans, attending meetings, having an active role on Person Centered reviews, etc. What we expect from our staff members: Have excellent communication skills, and ability to build professional relationships with the people we support, their families, etc Be positive and passionate about your work Compassion, dedication and flexibility Being able to work in a team or on their own Able to deal with emergency situations and conduct yourself in a professional way To liaise with Manager and office daily and to report any issues promptly.
