Practice NurseJob Vacancy. Job Title: Practice Nurse. Hours: Full Time 30-37.5 hours over 5 days;. (Ideally to include one regular extended hours session per week). Salary: £26,041 - upto £31,500 depending upon qualifications, experience and existing banding. The practice has its own pay scale and does not use the agenda for change pay rates. Undertaking routine nursing duties inline with RCN band 6 responsibilities. This is a senior post within our Practice Nursing Team working alongside our 2 other senior Nurses who collectively supervise the work of the Band 5 Nurses and Health Care Assistants. The post holder is responsible for the delivery of general practice nursing to the whole practice population, with a particular focus on Long Term Conditions Management. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required. As one of our senior Nurses, it is essential that this Nurse has primary care nursing experience and an understanding of the challenges of Primary Care and Community Nursing. We are not looking for this to be a training post, and as such are looking for someone with most (if not all) of the relevant long term conditions qualifications and associated experience. The team:. Leigh View Medical Practice is a large and still growing GP practice in Tingley on the Wakefield/Leeds border offering GP services to 16300 patients who live in Tingley and East Ardsley. We have a relatively small nursing team which we are currently redesigning to meet the needs of our growing practice population, and implement new projects. We employ 35 people in total and operate a recruitment policy in line with good equal opportunities practice. The primary needs of the practice are for someone suitably qualified and experienced in chronic disease management as well as traditional nursing duties. The criteria:. As a Band 6 equivalent Nurse, the successful candidate will meet the criteria below:. Knowledge. Knowledge of management of patients with long-term conditions. Knowledge of accountability of own role and other roles in a nurse-led service. Knowledge of local and national health policy and wider health economy awareness. Knowledge of patient group directions and associated policy. Skills. Clinical leadership skills. Clinical skills - immunisation and vaccination, ear care. Change-management skills and ability to support patients to change lifestyle. Excellent communication skills, both written and verbal. Negotiation and conflict management skills and ability to provide teaching and mentorship in a clinical setting. IT skills. Experience. Minimum 3 years post registration experience - essential. At least 2 years recent primary care nursing experience - essential. Experience of nurse-led management of long-term conditions. Experience of implementing protocols and clinical guidelines. Experience of quality initiatives such as clinical benchmarking and audit experience. Qualifications. Registered first level nurse. Diploma in Diabetes. Diploma in Asthma. Diploma in Cardiovascular Disease. Mentor/teaching qualification / Clinical supervision training and experience - desirable. Other. Self-directed practitioner, highly motivated.
