Role: CHC Case Co-ordinatorDuration: 3 Months with the view to extendLocation: South East EnglandRate: £250 - £300 a dayOur NHS client has a requirement for a Continuing Healthcare Case Co-ordinator for an initial 3 month contract.ESSENTIAL REQUIREMENTSMust be a Registered Nurse with current NMCExcellent verbal and written communication skillsExperience in the use of IT, databases and Microsoft OfficeKnowledge and experience of NHS Funding Continuing Healthcare critieriaExperience of developing care packages and assessmentsThe Role:You will be responsible for the co-ordination of assessments of individuals to identify their eligibility for NHS healthcare in line with the National Framework for Continuing Healthcare and Funded Nursing Care.The successful post holder will be working closely with hospitals, community services and care homes and facilitate a smooth assessment and referral process.You will also regularly review the clinical needs of the patient in regards to appropriateness or the placement, level of care being provided and entitlement to NHS Funded Healthcare.This team covers Surrey.
