Based at Park House Health &Social Care Centre, Carlton, Nottingham The Adult Community Service is an integrated healthcare team providing long-term conditions management, rehabilitation &community nursing services in Nottingham North &East. We are an enthusiastic team with a breadth of experience and a shared goal to offer the highest standard of care to our patients. We work closely with other health and social care professionals across primary &secondary care, as well as with patients, families and carers in both a home and clinic setting. We are looking for a Community Support Worker to join our dedicated team who is committed &passionate about promoting health, well-being and independence. We are a values-based Trust and are looking for dedicated and enthusiastic clinicians who believe in the provision of patient-centred care. A key aim of our service is to reduce unplanned admissions &facilitate discharges from the acute care environment. The team members include physiotherapists, occupational therapists, community nurses, matrons, specialist nurses and support workers, also working closely with GPs and voluntary sector. Your role will include both nursing and therapy elements and you will be expected to support patients in their own homes with exercise programmes, personal care, meal preparation, medication prompts and promoting independence. You should possess excellent interpersonal and time management skills, be a good communicator and able to prioritise and work well as a member of a team. An induction period and supernumerary support will be given. The service currently runs from 8am – 8pm, 7 days a week, therefore some weekend and bank holiday working may be required.
